Linkages Program
The aim of Linkages Programs is to provide support for people with complex and multiple support needs to remain living at home or in their local community. This includes all age groups, children and adults with a disability, frail older people and their carers. Linkages case managers assist clients and carers to develop care plans and co-ordinate services to meet client/carer needs. These services may include home nursing, respite, personal care, meals, podiatry or other services depending on the client/carer needs. Case managers provide advocacy and support and have ongoing contact with clients and carers.

Early Choices (0-6 Years) and Making A Difference (6-65 Years)
These programs assist families with children/adults with moderate to severe and/or multiple disabilities, to care for their family member at home. Families are linked into existing services, offered case management and support.

Additional assistance with services and equipment can be provided through a discretionary fund. Regionally, these programs are operated co-operatively by three agencies.
