Early Choices

Families can now receive support in the earlier stages rather than waiting until further down the track
 

image-case-mgt-03What is the Early Choices Program?

Early Choices is a family centred program which provides case management and support to families caring for a child (0-6 years) with a severe disability and/or very high support needs.

Our Case Managers work in partnership with families to identify their needs in relation to their child and to find ways of meeting those needs.

A family may be linked with existing services or funds may be available to buy services to meet particular needs. Services provided through Early Choices will be flexible and organised in ways which best suit these needs.

What does Early Choices Provide?

  • Information & referral
  • Co-ordination of services 
  • Family support & counseling
  • Advocacy
  • Planning for future needs

Where additional supports are required some funds may be used to purchase services and/or contribute to the cost of equipment, modifications and other items such as:

  • Additional assistance in the home at peak times, eg. bath time, meals, shopping
  • In & out of home respite to give families a break
  • Professional support, eg counseling, therapy services
  • Specialised aids & equipment
  • Household & vehicle modifications

Who is eligible?

A family may be eligible for Early Choices if they have a child who:
  • Has a severe disability
  • Has high support needs and
  • Is under school age *

*Families whose children are over school age may be eligible for the Making a Difference Program
