What is Case Management Support Services?
MOIRA provides person centered Case Management and support services for people with moderate to severe disabilities and/or their families. The impact of disability varies and many people find they cannot readily obtain the services they require to meet their needs.
A case manager works in partnership with people with a disability and their families to identify specific needs and assist in co-ordinating a range of services and supports.
These supports may include:
- Information and referral
- Co-ordination of services
- Family Centered Support
- Planning for future needs

range of services within the Southern Metropolitan Region.
Who is eligible?
You may be eligible for a Flexible Support Package if:
You have a family member living at home aged between 0 and 65 years and your family
member has a moderate to severe disability and/or complex support needs and live in the
local government areas of Stonnington, Port Phillip, Glen Eira, Bayside & Kingston.
Disabilities may include:
Physical, Intellectual, Sensory Neurological impairment or Acquired Brain Injury.
